Welcome New Founders

We are delighted that you became a member of our Founders Club. Plans are well underway for our Annual Founders Celebration – it is truly a spectacular and fun event that you will not want to miss. If you have not already done so, please be sure to mark your calendar for Saturday, March 20 at the Fontainebleau Resort. Your formal invitation should arrive within the next few weeks and we look forward to your RSVP.

Now, we request that you take a few minutes to help us appropriately recognize you as a new Founder and install you into our philanthropic family:

• Please complete the biography form so that we may include you in our Founders Ball program, a lasting memory of your philanthropic commitment. Should you prefer to write your own bio rather than fill out the form, please keep it under 125 words. Send your bio or the completed bio form via regular mail (Mount Sinai Medical Center Foundation, Attention: Mari Couvertier, 4300 Alton Road, Suite 100, Miami Beach, Florida 33140) or by email to rsvp@msmc.com.
  
  
• We also need a photograph of you to accompany your biography in the program. Please submit a head shot, either by regular mail or email as a .jpg or .pdf, to the address noted above. If you do not have a photo, we would be happy to take one at the Foundation office. Please contact Mari Couvertier at 305-674-3943 to schedule an appointment.
  
  
• Lastly, please take this opportunity to register for the Foundation online community. Joining the community affirms your commitment, allows you to manage your personal information, and facilitates future event registrations, donations and pledge payments.

Please note February 12, 2010 is the deadline for submitting all of this information. If we do not hear from you by then, we will assume that you do not want to be included in the Founders Ball program.
